چکیده

The patient discharge process is one of the most critical and complex healthcare processes as it involves several actors with a variety of skills and roles working towards the outcome of effective and efficient discharge of a patient from a hospital. This paper used a case study of the patient discharge process of two care units in a large urban hospital in which we analysed the process as an activity system using activity theory methodology. Through the case study we collected data from 63 nonparticipant observations interviews, 5 semi-structured interviews and document analysis. We analysed the data with the Activity-Oriented Design Method (AODM) and revealed eighteen contradictions in the discharge process activity system. The contradictions include information or documentation issues but also organizational and process issues, emphasizing that Hospital Information System (HIS) design for complex processes is a sociotechnical endeavor. Addressing these contradictions is an initial stage for systems analysis for the design of a health information system to improve the discharge planning process.

Connections, Communication and Collaboration in Healthcare’s Complex Adaptive Systems; Comment on “Using Complexity and Network Concepts to Inform Healthcare Knowledge Translation”

A more sophisticated understanding of the unpredictable, disorderly and unstable aspects of healthcare organisations is developing in the knowledge translation (KT) literature. In an article published in this journal, Kitson et al introduced a new model for KT in healthcare based on complexity theory. The Knowledge Translation Complexity Network Model (KTCNM) provides a fresh perspective by mak...

Priority Setting Meets Multiple Streams: A Match to Be Further Examined?; Comment on “Introducing New Priority Setting and Resource Allocation Processes in a Canadian Healthcare Organization: A Case Study Analysis Informed by Multiple Streams Theory”

With demand for health services continuing to grow as populations age and new technologies emerge to meet health needs, healthcare policy-makers are under constant pressure to set priorities, ie, to make choices about the health services that can and cannot be funded within available resources. In a recent paper, Smith et al apply an influential policy studies framework – Kingdon’s multiple str...

Translating Evidence into Healthcare Policy and Practice: Single Versus Multi-Faceted Implementation Strategies – Is There a Simple Answer to a Complex Question?

How best to achieve the translation of research evidence into routine policy and practice remains an enduring challenge in health systems across the world. The complexities associated with changing behaviour at an individual, team, organizational and system level have led many academics to conclude that tailored, multifaceted strategies provide the most effective approach to knowledge translati...

The Paradox of Intervening in Complex Adaptive Systems; Comment on “Using Complexity and Network Concepts to Inform Healthcare Knowledge Translation”

This commentary addresses two points raised by Kitson and colleagues’ article. First, increasing interest in applying the Complexity Theory lens in healthcare needs further systematic work to create some commonality between concepts used. Second, our need to adopt a better understanding of how these systems organise so we can change the systems overall behaviour, creates a paradox. We seek to m...
